Now for what you came for!
1. Paste the following code:
<input type="checkbox" name="option1"> Unchecked<br>
<input type="checkbox" name="option2" checked> Checked<br>
<input type="checkbox" name="option3"> Unchecked<br>

2. When you're creating a new box, paste a new line. Change option# to a number higher than the last. For an example, if the last number was 3, you'd change it to 4. To make it checked, add checked to the end of the code.
