Now for what you came for!
2. Use the Rectangle Tool
to paste a white box around the image like below (don't worry about the size). Change the opacity, Rasterize the shape layer, then delete any white part covering your image.

3. Change the opacity back to 100. Add a stroke to the white box, and it SHOULD add a border around your image as well. Make it black and size 2.

4. Now delete off the sides, bottom, or top until it looks like below.

5. Add some font to your image. Merge all the polariod layers together. Copy and paste it into a new file, then resize by about 75%. Copy and paste it back, then rotate.
